Fixed BT48x 64x64 hardware cursor bugs (caused by the index variable being uint8_t when it should have been uint16_t) introduced in the previous commit.
Changed the 964-based Stealth64 BIOS to use the 1.07h BIOS and report an ID of D0 instead of D1, because the D1 revision appears to have changes that are not documented anywhere.

Rewritten the ICD2061 clock chip and moved the Diamond Stealth 32 back to the main branch as it's now fixed;
Fixed the Y offset of the 64x64 hardware cursor of the Cirrus Logic cards.

Added internal graphics card option for the Intel Advanced/EV for the on-board S3 Trio64;
Fixed configuration for internal graphics card options (using the machine's Configure button) for some machines (PCjr, Tandy 1000, Tandy 1000 HX, and now also the Intel Advanced/EV).
IBM.H is gone, video stuff re-organized. Keyboard stuff reorganized.
Machines that have their own video, mouse and/or keyboard now have all this in their machine file.
Fixed and other cleanups here and there.
Changed the CD-ROM capabilities MODE SENSE page a bit;
Applied the S3 Trio and SVGA changes from PCem;
Fixed saving of the video renderer settings;

The Settings dialog now correctly filters devices by bus;
Split the dual VLB/PCI graphics cards into VLB and PCI versions;
Device filtering is now done using the new device_is_valid() call that compares device flags to machine flags;
Moved the NCR-based SCSI controllers to the main branch;
Moved the NE1000 to the dev branch until it's finished.
