Moved the BT48x hardware cursor draw function to the RAMDAC's code where it belongs;
Renamed the BT48x RAMDAC code file to vid_bt48x_ramdac.c (was vid_bt485_ramdac.c);
Fixed BT48x 64x64 hardware cursor bugs (caused by the index variable being uint8_t when it should have been uint16_t) introduced in the previous commit.

Re-worked the ATI 28800 a bit, added Sierra SC1502x ramdac to VGA Wonder XL (Compaq) since it supports such.

Reimplemented saving of all the non-volatile things of the Toshiba T1000 and T1200;
Applied the latest T1200 patch from John Elliott;
Applied the latest ATI Korean VGA patch from greatpsycho.
The pages array are no longer allocated for the entire memory of space if memory size + 384 is above 16 MB (turns out it was not needed at all), further significantly reduces the emulator's RAM usage;
Made greatpsycho's revised SVGA status handling specific to ATI cards, while the generic (S)VGA code uses the old handling - fixes error 2401 on PS/2 Model 80.
Turned the char arrays in gameport.h into const char pointers;
The makefile no longer links in the (unfinished) PIIX4 emulation code (which is currently not used by anything even in the Dev branch).
IBM.H is gone, video stuff re-organized. Keyboard stuff reorganized.
Machines that have their own video, mouse and/or keyboard now have all this in their machine file.
Fixed and other cleanups here and there.
The Settings dialog now correctly filters devices by bus;
Split the dual VLB/PCI graphics cards into VLB and PCI versions;
Device filtering is now done using the new device_is_valid() call that compares device flags to machine flags;
Moved the NCR-based SCSI controllers to the main branch;
Moved the NE1000 to the dev branch until it's finished.
