Various portability fixes mosty for MSVC which doesn't have drand and

doesn't allow dynamic local arrays.
v_fragment -> v_fragment_t
libcdio.sym: more external symbols defined
/lib/_cdio_linux.c: harmless type mismatch
example/sample2.c -> example/device.c
