729b48f1a8 Go back to two-digit format for minutes in MSF. CD's really can't have 
more than 99 minutes in them. So we shouldn't give the illusion they
can.

0441b2f09d CAM mode2 form1 and form2 reading fixes. 
Default device lists default to CAM device name.
Simplify, correct and make more complete CAM support.

With the above we can finally use libcdio for cd-read, vcdimager and
probably vlc and xine.

2a54d504c0 MSWindows/*: get access mode working better. ASPI support is faulty though 
freebsd.c: it's "ioctl" not "IOCTL";
cdrdao: it's cdrdao, not "toc"
cd-info: slightly better error message?

cb7c0e870a Go over FreeBSD code based on output from Heiner. 
Hopefully 3 bugs are fixed:
  - we get the leadout track now, no core dumps
  - MSF reporting is corrected
  - track format and mode

_cdio_linux.c: use cdio #define rather than GNU/Linux one.
