Add CDIO_MAX_DRIVER (distinct from CDIO_MAX_DEVICE_DRIVER

cdio.c (cdio_open): was only scanning devices. Change to scan
disk image files as well.

d0999c5ebc With M$ we can no longer assume we can open a fd ant it will read 
properly. Add yellow-mode reads interface.
