We have 115 direct inclusions of "system/memory.h", and 91 headers
in include/ use it: hundreds of files have to process it.
However only one single header really uses the MemoryRegionCache
API: "hw/virtio/virtio-access.h". Split it out to a new header,
avoiding processing unused inlined functions hundreds of times.

Commit 500131154d ("exec.c: Add new address_space_ld*/st*
functions") added a new API to fix a shortcoming of the
ld/st*_phys() API, which does blind bus access, not reporting
failure (and it also allow to provide transaction attributes).
Later commit 42874d3a8c ("Switch non-CPU callers from ld/st*_phys
to address_space_ld/st*") automatically converted the legacy uses
to the new API, not precising transaction attributes
(MEMTXATTRS_UNSPECIFIED) and ignoring the transation result (passing
NULL pointer as MemTxResult).
While this is a faithful replacement, without any logical change,
we later realized better is to not use MEMTXATTRS_UNSPECIFIED or
NULL MemTxResult, and adapt each call site on a pair basis, looking
at the device model datasheet to do the correct behavior (which is
unlikely to ignore transaction failures).
Since this is quite some work, we defer that to device model
maintainers. Meanwhile we introduce a definition, to allow a
target which removed all legacy API call to prohibit further
legacy API uses, named "TARGET_NOT_USING_LEGACY_LDST_PHYS_API".
Since all targets should be able to check this definition, we
take care to not poison it.

stl_phys_notdirty() was added in commit 8df1cd076c ("physical memory
access functions") as a (premature?) optimisation for the CODE path.
Meanwhile 20 years passed, we might never have understood / used it
properly; the code evolved and now the recommended way to access the
CODE path is via the cpu_ld/st_mmu*() API.
Remove both address_space_stl_notdirty() and stl_phys_notdirty()
leftovers.

In our long-term experience in Bytedance, we've found that under
the same load, live migration of larger VMs with more devices is
often more difficult to converge (requiring a larger downtime limit).
Through some testing and calculations, we conclude that bitmap sync time
affects the calculation of live migration bandwidth.
When the addresses processed are not aligned, a large number of
clear_dirty ioctl occur (e.g. a 4MB misaligned memory can generate
2048 clear_dirty ioctls from two different memory_listener),
which increases the time required for bitmap_sync and makes it
more difficult for dirty pages to converge.
For a 64C256G vm with 8 vhost-user-net(32 queue per nic) and
16 vhost-user-blk(4 queue per blk), the sync time is as high as *73ms*
(tested with 10GBps dirty rate, the sync time increases as the dirty
page rate increases), Here are each part of the sync time:
- sync from kvm to ram_list: 2.5ms
- vhost_log_sync:3ms
- sync aligned memory from ram_list to RAMBlock: 5ms
- sync misaligned memory from ram_list to RAMBlock: 61ms
Attempt to merge those fragmented clear_dirty ioctls, then syncing
misaligned memory from ram_list to RAMBlock takes only about 1ms,
and the total sync time is only *12ms*.

Add IgvmNativeVpContextX64 struct holding the register state (see igvm
spec), and the qigvm_x86_load_context() function to load the register
state.
Wire up using two new functions: qigvm_x86_set_vp_context() is called
from igvm file handling code and stores the boot processor context.
qigvm_x86_bsp_reset() is called from i386 target cpu reset code and
loads the context into the cpu registers.

SPDM maybe used over different transports. This patch specifies the
trasnport types as an enum with a qdev property definition such that
a user input transport type (string) can be mapped directly into the
respective SPDM transportenum for internal use.

This header contains the transport encoding for an SPDM message that
uses the SPDM over Storage transport as defined by the DMTF DSP0286.
Note that in the StorageSpdmTransportHeader structure, security_protocol
field is defined in the SCSI Primary Commands 5 (SPC-5) specification.
The NVMe specification also refers to the SPC-5 for this definition.
The security_protocol_specific field is defined in DSP0286 and is
referred to as SP Specific for NVMe and ATA.

This is to support uni-directional transports such as SPDM over Storage.
As specified by the DMTF DSP0286.
Also update spdm_socket_rsp() to use the new send()/receive() functions. For
the case of spdm_socket_receive(), this allows us to do error checking
in one place with the addition of spdm_socket_command_valid().

QEMU maps certain regions into the guest multiple times, as seen in the
trace below. Currently the MSHV kernel driver will reject those
mappings. To workaround this, a record is kept (a static global list of
"slots", inspired by what the HVF accelerator has implemented). An
overlapping region is not registered at the hypervisor, and marked as
mapped=false. If there is an UNMAPPED_GPA exit, we can look for a slot
that is unmapped and would cover the GPA. In this case we map out the
conflicting slot and map in the requested region.

The mapping table is guarded by a mutex for concurrent modification and
RCU mechanisms for concurrent reads. Writes occur rarely, but we'll have
to verify whether an unmapped region exist for each UNMAPPED_GPA exit,
which happens frequently.

Connect the x86 instruction decoder and emulator to the MSHV backend
to handle intercepted instructions. This enables software emulation
of MMIO operations in MSHV guests. MSHV has a translate_gva hypercall
that is used to accessing the physical guest memory.
A guest might read from unmapped memory regions (e.g. OVMF will probe
0xfed40000 for a vTPM). In those cases 0xFF bytes is returned instead of
aborting the execution.

Create MSHV vCPUs using MSHV_CREATE_VP and initialize their state.
Register the MSHV CPU execution loop loop with the QEMU accelerator
framework to enable guest code execution.
The target/i386 functionality is still mostly stubbed out and will be
populated in a later commit in this series.

Add memory listener hooks for the MSHV accelerator to track guest
memory regions. This enables the backend to respond to region
additions, removals and will be used to manage guest memory mappings
inside the hypervisor.
Actually registering physical memory in the hypervisor is still stubbed
out.
