AUD_init_time_stamp_{in,out} and AUD_get_elapsed_usec_{in,out} are only
used by the adlib device. The result isn't actually being used since
ADLIB_KILL_TIMERS was set some 20y ago. Let's drop this dead code now.
Drop QEMUAudioTimeStamp as well as reported by Akihiko Odaki.

Most handlers implementations has name like {unit_name}_{handler_name},
which is usual and well-recognized pattern. Convert the rest (especially
with useless qemu_ prefixes and misleading qmp_ prefixes) to the common
pattern.

All the functions are about "-audio model=" handling, a simpler
way to setup audio. Rename functions/variables to reflect this better.
audio_register_model_with_cb() dropped "pci" from the name, since it
will be generalized next.
deprecated_register_soundhw() was actually not a function to be
removed since it's used for "-audio model=" aliasing.

Headers in include/sysemu/ are not only related to system
*emulation*, they are also used by virtualization. Rename
as system/ which is clearer.
Files renamed manually then mechanical change using sed tool.

I'm not documenting every single change in the codeconverter
script because most of that code will be deleted once we finish
the QOM code conversion. This patch updates the script to the
latest version that was used to perform changes in the QOM code.

This started as a simple script that scanned for regular
expressions, but became more and more complex when exceptions to
the rules were found.
I don't know if this should be maintained in the QEMU source tree
long term (maybe it can be reused for other code transformations
that Coccinelle can't handle). In either case, this is included
as part of the patch series to document how exactly the automated
code transformations in the next patches were done.
